Appointments & Consultations 

The phone consultation and the appointment call can be taken care of in one phone call if you choose.  If you are calling for a consult only, please follow just the calling instructions below.

The following steps must be taken in order to make an appointment for your reversal surgery. Please be sure you read this entire page to make sure you have followed the directions carefully for your consultation and appointment confirmation.

Steps to making/confirming your appointment:

- Choose your surgery date from the calendar
- Call Dr. Levi for pre-op consultation and to confirm the appointment date.
- Email your contact information to the appts desk and request your invoice
- Make your deposit.
- Schedule your transportation through shuttle page (if needed)

1. Choose your date

Please see the calendar for choosing a date that is best for your reversal surgery if you do not have a preferred date already.  If you are able to pay your deposit immediately, we can schedule with as little as 1 week's notice.  Information below may change so please check often for updates.  Note - these are general times and guidelines; occasional exceptions and changes may be made.

Currently scheduling through June 2009:

Monday - Thursday - 1 reversal surgeries (7pm)
Friday -
 2 reversal surgeries (7pm & 9pm)
Saturday -
4 reversal surgeries  (7am-afternoon)
Sunday -
2 reversal surgeries (7am-noon))

2. Call "Dr. Levi"

Dr. Levi Diaz is assistant surgeon & liaison for international patients.  He speaks fluent English / Spanish.  Tell him the date you desire, confirm availability and schedule your reversal surgery.   This call will serve as your initial consultation, so please

  • ask all questions you have concerning your  surgery

  • inform Dr. Levi of any previous medical conditions, allergies, height, weight, pre-existing illnesses, previous surgeries, drug/latex allergies, and so on

  • call during consultation hours only

With the exception of medical emergencies, all calls should be during the hours of..  4-8 pm (CST) weekdays / or anytime on weekends

3. Email the office

This step is important!  We will need to reach you by email!   Even if you give your contact information during your call, you must email the appointment office with your contact information and request your deposit invoice and pre-op evaluation sheet.  If your information changes, please notify us immediately.

It is your responsibility  to contact us, if not, your appointment will be cancelled after  7 days without notice.

4. Make your deposit

Deposits should be made immediately following your appointment call.  You must request an invoice by email.  We will not contact you first, you will have to notify us. 

You appointment is not complete until we have received your contact information by email and your deposit is made.  See info page below. If you schedule and do not make a deposit, you may be charged a missed appointment fee to reschedule. 

 

CANCELLATION/RESCHEDULING GUIDELINES: 

please read carefully!

CANCELLATIONS  To cancel your appointment for any reason, contact us via email.   A courtesy call or email is expected  in advance, allowing us time to schedule someone from the waiting list.  If you miss your appointment without advance notice* you will be charged a $150 missed appointment fee before you are allowed to reschedule.  For deposit refunds 30+ days notice of cancellation is required.  See the deposit page for complete guidelines on deposit refunding.  (*30 days if you cancel the appointment; 2 weeks if you are rescheduling).

RESCHEDULING:    If you need to reschedule your appointment, you can do so by emailing us with your changes.  You must give at least  2 weeks notice and tell us your new date at the time.  The new date must be within 90 days or your original date for your deposit  to apply.  You may schedule 2 appointments on one deposit in a 6 month period.  All third appointments require a new deposit. No grace period is given for second invoice payments.   You will not be added to the surgery schedule until the deposit is paid. We will work with you on this anyway we can.

OTHER FEES:  Patients who schedule their appointments more than once and DO NOT  follow through by paying their deposits may be charged a rescheduling fee of $150 before being allowed to reschedule.  This fee may be waived one time depending on the circumstances.  Patients who make appointments and /or do not show up for surgery will be charged a missed appointment fee of $282.50 before being allowed to reschedule.  If you are not sure you can make the deposit please do not make the appointment or let us know in advance  Holding dates that you are not able to pay for is not fair to the other surgeons, the hospital or the other patients who need the time.   

VERIFYING YOUR APPOINTMENT  If we have your correct information, you will receive an email from us the week of or before (generally Monday's) your surgery to confirm your appointment.  This email will also contain pre-op instructions you will need to follow before arriving at the hospital. If you do not receive it, we may not have your correct email information, so email us ASAP if you have not received your pre-op instructions.  If we do not hear from you the week before your reversal date, we may assume you are not coming.

Two very important details...

One We respectfully ask that you do not make your appointment if you are unsure about such things as the date, your funds, child care, finding a guest to travel with, time off from work pre/post op and various other issues.   This is unfair to others who may need the date that you are not able to keep or that you are not well prepared for;   it is also an inconvenience to many on our staff and our doctors/surgeons who must arrange their schedules around your appointment.   Should 'unexpected'  problems arise,  please reschedule ASAP or contact us for suggestions.  We are more than happy to help. 

Two Because we are out of the country and have to rely mostly on email, it is the patient's responsibility to make sure we are notified by email with your contact information and changes in scheduling; and to make sure that all fees are paid in the appropriate time frames.   If we cannot find you, we cannot work with you to make sure your obligations are fulfilled.  So emailing the office is a MUST.
