How to make/confirm your appointment:

  1. Decide your surgery date

  2. Call or email to make the appointment (below)

  3. Request your invoice and pre-op papers

  4. Pay deposit and send your paperwork

  5. Schedule your transportation shuttle (if needed)

Please be prepared ~ have your deposit ready to pay at the time you make your appointment.

Making Appointments:  choose whichever way is most convenient for you

If you have medical records (not mandatory but helpful) you can send them to us by email or fax.  Please fax ONLY the ‘procedure description’ portion of your records - usually a 1-2 page typed note describing the ligation). After the doctor reviews them we will give his report by email.

1) Email:  you can to make your appointment by email for convenience, just email for your paperwork and let us know the date of your choice.

2) Phone:  if you prefer to discuss your surgery with a doctor in advance you should call to make the appointment.  This is your consultation call and you can make your appointment at the same time.  011-52-899-871-1586 - or - 011-52-899-132-2614 - daily (including weekends) 8am-8pm central time

Pre-Op instructions:  If we have your correct information, you will receive your pre-op instructions approximately 5-7 days prior to surgery.  Please email us ASAP if you have not received your  instructions a few days before your appointment.  If we do not hear from you the week before your reversal date, we may assume you are not coming.


Choosing your date:

Please choose a date that is best for your reversal surgery.  If you are able to pay a deposit immediately, we can schedule with as little as one week's notice - otherwise try to allow at least 2 weeks.  The calendar listed below will show any open/closed dates.    Note - these are general times and guidelines; occasional exceptions and changes may be made.   Currently scheduling to the end of 2010.  Information below is subject to change so please check back.

Mon - Fri 3 reversal surgeries (1-3pm, 7pm & 9pm)

Calling for consultation / Emailing the office

Our physicians speak fluent English / Spanish.  If you call to make your appointment, be sure to ask any questions you may have.  A pre-op evaluation form will be sent upon scheduling or in advance if requested - the evaluation will allow the doctor to give you an  estimated cost for your surgery.  All patients must fill out the evaluation form.

Important!  Even if you give your contact information during your call, you must email the office with your contact information and request your deposit invoice and pre-op papers. If your information changes, please notify us immediately.  It is the patient's responsibility  to maintain contact with our office.  If we are unable to contact you, the appointment may be cancelled without notice.  All email correspondence is in English unless otherwise requested. 

You appointment is not complete until we have received your contact information by email and your deposit is made.  See info page below. If you schedule and do not make a deposit, you may be charged a missed appointment fee to reschedule. 

CANCELLATION/RESCHEDULING GUIDELINES:  please read carefully!

CANCELLATIONS

  •  To cancel your appointment for any reason, contact us via email.   A courtesy call or email is expected  in advance.   30 days minimum notice for cancellations. 

  • If you miss your appointment without notice* you will be charged a rescheduling fee (equal to half of the deposit).   See the deposit page for complete guidelines. 

RESCHEDULING:   

  •  If you must reschedule, please email us with your changes.  2 weeks notice is required for rescheduling.

  • A new date must be chosen at the time you reschedule and must be within 90 days of original date for deposit to apply. 

  • You may schedule 2 appointments on one deposit (initial appt + 1 reschedule). 

  • All subsequent appointments require a rescheduling fee equal to 50% of the deposit, or a full deposit depending on the circumstances.

OTHER FEES: 

  • Patients who schedule appointments and DO NOT  follow through with confirming the appointment more than once will be charged a rescheduling fee in addition to the deposit.  This fee may be waived one time depending on the circumstances. 

  • Patients who make appointments and do not show up for surgery will be charged a missed appointment fee in addition to the deposit before being allowed to reschedule. 

  • If you are not sure you can make the deposit within 7 days of your call,  please do not make the appointment

The above guidelines for rescheduling/cancellations only apply to new patients who do not have a history of scheduling/canceling surgery dates. 

2 very important details...
We
respectfully ask that you do not make your appointment  if you are unsure about the date, your funds, child care, time off work and various other issues.   This is unfair to other patients and very inconvenient to the hospital staff and other doctors with patients who willingly arrange their schedules around international travelers.  We understand that 'unexpected'  problems can arise,  but please contact us immediately for assistance if that happens.  Please remember not to make unchangeable travel plans until your appointment has been confirmed.   We cannot schedule your surgery 'around' your travel plans. 

Because we rely mostly on email communication, it is the patient's responsibility to make sure we have your contact information.  If we cannot find you, we cannot work with you - so keeping in touch with the office is mandatory.
